Completion of this form acknowledges that my district will pay $1,750.00 per registered participant. Refreshments and light meals are included.
I understand the cost of travel and expenses to attend meetings is the responsibility of my district and should be coordinated with the participant(s).

Submitters will receive an email in August 2018 to confirm team members and update information for
any individual indicated as TBD. Once team members are confirmed, individuals will receive a welcome email with detailed DxD
